Credentialled Diabetes Educators (CDEs) are health professionals with specialist training in the management of diabetes. They provide education, support, and practical strategies to help you understand your condition, manage your treatment, and feel confident in daily life.

Tailored Diabetes Support Programs
Newly Diagnosed Type 1 Diabetes
We offer a comprehensive treatment program for those newly diagnosed with Type 1 diabetes. Our multidisciplinary team of endocrinologists, diabetes educators, and dietitians work closely together to provide:
Immediate support and education following diagnosis
Insulin initiation and dose adjustment
Carbohydrate counting and food education
CGM and pump selection, application, setup and training (if appropriate)
Support throughout the adjustment period
This structured program ensures each patient receives timely, coordinated care from the very beginning.
Newly Diagnosed Type 2 Diabetes
Our team-based approach helps people with Type 2 diabetes understand their condition and take positive action early. The program includes:
Education on how diabetes develops and what influences glucose levels
Guidance on nutrition, physical activity, and medication (if required)
Blood glucose monitoring techniques
Goal-setting and long-term care planning
Support is provided by a combination of endocrinologist, diabetes educator, dietitian and exercise physiologist to establish a personalized care plan that supports lifestyle change and complication prevention.
Transition Support for Adolescents Living with Type 1 Diabetes
Moving from paediatric to adult care is a significant step, especially for adolescents and young adults living with Type 1 diabetes. This stage of life brings new pressures, responsibilities, and lifestyle changes that can make diabetes management more challenging.
At LDE, we offer a dedicated transition program to support young people as they leave Perth Children’s Hospital and begin their journey in adult private care.

With the patient’s consent, we welcome family members or support people to attend appointments. Their involvement can be especially helpful during times of transition, while we continue to support their independence and confidence in managing their care.
To make life easier for busy students and families, we can also arrange back-to-back appointments across disciplines so you can see multiple team members in a single visit and save valuable time. Telehealth appointments are available after the first session.
GDM (Gestational Diabetes Mellitus) Care
Gestational diabetes requires prompt, sensitive, and informed care. Under the supervision of an endocrinologist, diabetes educator and dietitian, we provide:
Rapid appointment availability for timely treatment
Education on blood glucose monitoring and safe nutrition
Insulin initiation and titration if needed
Ongoing reviews throughout the pregnancy

Complications Prevention & Risk Reduction
Prevention is better than cure. We provide guidance on foot care, hypo glycaemia management, cardiovascular risk, and when to seek medical review. Our aim is to protect your health through proactive, knowledge‑based self‑care.

We understand that managing your health needs to fit into your life. That’s why we offer both in-clinic appointments and telehealth consultations, so you can access expert support wherever you feel most comfortable.
Whether you're at home, at work, or living regionally, our telehealth option ensures you receive the a high standard of care via secure video or phone.
All telehealth services are eligible for Medicare or private health rebates under the same conditions as in-person consultations, with a valid GP referral.

Rebates may be claimable via:
Medicare (with a valid GP Management Plan)
Private Health Insurance (check with your provider)
A GP referral is required to claim a rebate under a GP Management Plan.
