Our Cardiometabolic Assessment Clinic brings together experts from Western Cardiology and Living Diabetes & Endocrinology to provide you with a comprehensive evaluation of your heart and metabolic health.

What is metabolic health and why is it important?

Cardiometabolic health refers to the overall well-being of your cardiovascular system (heart and blood vessels) and metabolic system (how your body processes energy). It encompasses factors like blood pressure, cholesterol levels, blood sugar, body weight, and the absence of diseases like diabetes or heart conditions.

Maintaining good cardiometabolic health is crucial because it reduces the risk of serious conditions such as heart disease, stroke, and type 2 diabetes. These are among the leading causes of death globally. By focusing on cardiometabolic health, you can improve your energy levels, maintain a healthy weight, and enjoy a longer, healthier life.

Which Doctors will provide the assessment?

Our team includes both an endocrinologist and a cardiologist working together to assess your risk factors and associated medical conditions to create a personalised management plan to support your health and future well-being.

Why should I be referred to a Cardiometabolic Assessment Clinic?

When you treat all aspects of your cardiometabolic health together under one plan it gives you a better chance of reducing your future risk. We provide a cardiometabolic assessment when you have any combination of the following health concerns:

High blood pressure High Cholesterol Known Heart Disease

Type 2 Diabetes Kidney Disease Sleep Apnoea Fatty Liver Disease

Overweight/obese Metabolic syndrome Prediabetes

Each of these factors raises your risk for heart disease, and together they significantly increase the chance of severe cardiovascular issues like heart attack and stroke.

Early diagnosis and treatment can reduce your risk and prevent the progression of cardiovascular disease.

What to expect when you’re referred for a cardiometabolic assessment?

You will receive one referral from your GP covering your full cardiometabolic assessment. The team at Living Diabetes & Endocrinology will make both appointments for you, if we receive the referral directly from your GP we will contact you. Otherwise please contact our friendly team on 08 6374 6438 to organise your appointments.

Your initial appointment will be with our endocrinologist at Living Diabetes & Endocrinology followed by an appointment with our cardiologist at the same location. Your team will then collaborate during a multidisciplinary meeting with our allied health professionals to develop a coordinated management plan which is sent to you and your GP.

What are the costs involved?

You will see both an Endocrinologist and a Cardiologist on separate days at LDE,  with the assessment planned to be completed within 4-6 weeks from the first appointment. The cost of the appointment will be at the discretion of the treating doctor and based on your personal medical circumstances.

The maximum cost for each appointment will be $420 with Medicare refunding $259.40, being a maximum total of $840 with an out-of-pocket expense of $321.20.

There is no cost associated with the multidisciplinary team meeting where your management plan is finalised.

What happens after the cardiometabolic assessment?

Following your cardiometabolic assessment you and your GP will receive a plan with recommendations on your future care, which will be under the supervision of the Endocrinologist and allied health professionals at Living Diabetes and/or the Cardiologists at Western Cardiology.

Your on-going health and wellbeing are of great importance to us and we will work with you and your GP to improve your cardiometabolic health.

Visit your GP to discuss this assessment and arrange for a referral.
